Who wrote/created the book?
I originally thought it was the Blue Fairy as she's lived very long and is involved with a few of the stories. She would easily have the knowledge to create the book.
But after looking at a youtube comment about Rumplestiltskin making the book that would work too. Rumple wants the curse broken as seen in Storybrooke. Obviously to search for his missing son in the real world. With all the powers he possess I could see him creating the book and enchanting it somehow or working it in the curse to save the book as well as himself. Then he made sure it got delivered to Mary Margaret who in turn gave it to Henry knowing he would eventually find Emma and bring her to Storybrooke to break the curse.
Of course it could still be someone else but The Blue Fairy or Rumplestiltskin making it makes the most sense to me right now.

I'm sticking with the blue fairy. The problem with Rumple writing the book is the writers confirmed he got his memories back when Emma came to town. (Big plot hole as far as I'm concerned because if that's the case, how did he get Henry, unless the blue fairy helped?)
So I think the blue fairy wrote the book and I think she never lost her FTL memories.

Well the thing is either could have written in the fairy tale world and enchanted it somehow off screen. Since a few magical objects survived from Fairy Tale World the book could have too.
The problem with Blue Fairy keeping her memories is she was only able to send two people to escape the curse. It looked like her powers were limited against the curse. While Regina enacted it and Rumple created it which is why they remembered. But we don't know if the Blue Fairy remembered before the finale though. I guess right now it still could be either one of them or someone else as well.

I think August identifying himself as a writer was a nod to the audience implying that he wrote the book. He was supposed to be helping Emma on her journey so he did so by possibly writing the book for Henry, and keeping Neal away from Emma.
He knows his way around binding books. He did put in the story about himself in the book, the pages did not seem out of place and there was no damage to the book whatsoever. Where would he get the same type of paper, use the same calligraphy and be able to make the same illustrations unless it was him who wrote it originally?

but if it was August why would he then need to take it from henry's hidding place to add more pages to it??
i somehow don't think its him well not him alone... at least...
not sure who it is just yet..

August couldn't have written the book. It would make no sense. How would he know all the stories and the details? He was a little kid when the happenings of the book took place back then in FTL. And as Nix pointed out, why wouldn't he have added his part of the story to begin with?
Furthermore, the author of the book doesn't seem to know Rumplestiltskin since Henry couldn't figure out who Mr. Gold was but August knew exactly who he was.
I also have a feeling it was Rumplestiltskin himself to make the breaking of the curse easier.

if that's so that's not only a big plothole but a lame one, he designed the whole damn thing he should be immune to it's effects from the getgo. Cora knew a lot about the curse though maybe she helped Rumpel create it to begin with?
